政策

To encourage and promote a safe work environment, an employee who experiences any work 相关的 accident, injury, and/or illness is required to report 的 information to 的 College in accordance with established procedures. 员工应该报告 的 incident(s) to 的 respective supervisor immediately or as soon as practically 可能的.

If 就医 is needed because of a work 相关的 accident, injury and/or illness, it is 的 employee’s responsibility to notify a physician that 的 incident is work 相关的. The employee may choose to see 的ir own medical provider or a medical provider that treats work 相关的 injuries and accidents. The College can recommend a medical provider that accepts work 相关的 injury patients.

If 的 accident, injury and/or illness results in 时间损失 that is compensable under 的

Washington State Workers’ Compensation Law, an employee must submit leave requests for time missed from work while off work due to a work 相关的 injury.  一个员工 who suffers a work 相关的 injury or accident is not eligible for shared leave.

  1. 一个员工 who is eligible for sick leave, vacation/annual leave, compensatory time, or holiday pay may elect to receive payments for any of 的se and for industrial insurance 时间损失. 
  2. Any payback to 的 state of sick leave used during 时间损失.
  3. 一个员工 may elect to be on leave without pay and receive industrial insurance 时间损失赔偿.

It is 的 responsibility of 的 supervisor to report and forward 的 completed Report of Employee Personal Injury/Incident/Accident form to Human 资源.

It is 的 responsibility of 的 College, 的 supervisor, and 的 employee to comply with 的 provisions of 的 College Return To Work 政策 if light or modified duty 是一个被批准的操作.

目的

The purpose of this policy is to emphasize 的 College’s commitment to provide a safe work environment and to outline 的 responsibilities and procedures for reporting work 相关的 accidents, injuries and/or illnesses.  This policy sets forth 的 responsibilities and procedures for claiming workers’ compensation due to an accident, injury and/or illness in accordance with College policy and state laws.

过程

一个员工 who has a work 相关的 accident, injury and/or 疾病应:

  1. Immediately report 的 accident, injury and/or illness to his/her immediate supervisor;
  2. Complete 的 College Report of Employee Personal Injury/Incident/Accident form  within 24 hours of 的 incident or as soon as practically 可能的.
  3. Complete 的 employee’s portion of 的 Washington State Fund Accident Report form (F242-130-000) if 的 accident, injury and/or illness requires 的 employee to seek 就医. The medical provider initiates 的 claim paperwork and provides 的 Washington State Fund Accident Report Form to 的 injured eimployee at 的 time of 的 reported incident for 的 employee’s completion.
  4. Complete leave requests and submit 的m through 的 TLS (Time and Leave System) electronic leave reporting system when absent from work.

请注意我&我向雇主提供通知 - Provide a release to work statement from a medical provider if 时间损失 has occurred because of a work 相关的 accident, injury and/or illness. 学院可以要求 a release to work statement or an activity prescription form from 的 employee even if 的 employee does not receive 时间损失.

The supervisor of an employee who has reported a work 相关的 accident, injury and/or 疾病应:

  1. Forward 的 completed College Report of Employee Personal Injury/Incident/Accident 表格送交人力资源部;
  2. Forward all work 相关的 accident, injury and/or illness employee documents received from 的 employee to Human 资源.
  3. Consult with 的 Human 资源 office to coordinate any return to work provisions 根据学院政策.
